South Carolina Solutions is a Medical Homes Network (MHN) Program which is responsible for delivering primary care case management services to eligible Medicaid beneficiaries.
A Medical Home Network (MHN) is composed of a Care Coordination Services Organization (CSO), in this case South Carolina Solutions, and Primary Care Physicians enrolled in our network. South Carolina Solutions supports the physicians and enrolled members by providing care coordination, disease education, and data management. The Primary Care Physicians manage the health care of their members, which includes authorizing services provided by other health care providers.
In South Carolina Solutions:
• Members keep all of their Medicaid benefits, PLUS, they get unlimited doctor visits
• Members have access to a nurse by phone 24 hours a day, 7 days a week
• Members have someone to help them get needed care
• If a member has a chronic illness, they have someone to help them learn how to manage their illness better.
The Primary Care Physician will:
• Provide most of the members' care
• Help members get the care they need outside of the primary care office
• Keep up with any special medical needs a member or member's child may have
• Help members stay healthy!
