Becoming a Provider

At South Carolina Solutions we are always available to answer questions from current and potential medical home providers. Please find some basic program information below. If you are interested in joining our network, please contact us at 1-888-366-6243 or email networking@sc-solutions.org .

Current network providers can also log in to the “Already a Provider” portal and obtain practice-specific reports, the South Carolina Solutions Provider Manual, and other valuable information.
