What you should know:
South Carolina Solutions is a Medical Homes Network (MHN) Program which delivers primary care case management services to people on Medicaid.
A Medical Home Network (MHN) is made up of a company like South Carolina Solutions that runs the program, and the doctors in our network that provide the care. South Carolina Solutions will help you and your doctor with the program by giving you information and helping you get services. Your doctor manages your health care, and also will send you to other doctors if you need it.
In South Carolina Solutions:
• You keep all of your Medicaid benefits, PLUS, you get unlimited doctor visits
• You have access to a nurse by phone 24 hours a day, 7 days a week
• You have someone to help you get the care you need
• If you are sick, you have someone to help you learn how to manage your illness better
Your doctor will:
• Provide your health care
• Help you get the care you need outside of the primary care office
• Keep up with any special medical needs you or your child may have
• Help you stay healthy!
